Crossroads are commissioned by GP’s and Social Services to provide a rapid response service, to help prevent the breakdown of a caring situation that might otherwise lead to a hospital or care home admission. By providing replacement care at a time of emergency or crisis, we aim to reduce the level of disruption and distress for people with care needs. Currently referrals can only come through Health or Social Care professionals, but if you feel you need this service, please ask a health or social care professional to refer you.
